If you do not have either of those, a copy of your mortgage or lease statement as well as one of the following must be provided: • Telephone Bill (Cell Phone bills will not be accepted) • Voter's Registration Card • Vehicle Registration Card • Current Utility Bill (ComEd, Nicor, Water/Sewer) • Village of Oak Park vehicle sticker receipt You may email to info@pdop.org, mail, or show a copy of this information at one of our registration locations. 1 Unless otherwise noted, refunds must be requested 48 hours before a program meets for the second time or no refund will be issued. is can now be done by phone! 2 Refund requests for trips, ticketed events, and one-day programs must be received two weeks before the program meets to be eligible for a refund. Refunds within two weeks will be considered only if the vacated spot can be filled by a wait-listed participant. For ticketed events that are full, no refund will be given. 3 A $3 cancellation fee will be charged for all refund requests unless otherwise stated. GYMNASTICS REFUND POLICY Refunds must be requested 48 hours before a program meets for the second time to be eligible. Refunds will be reduced by $10 service fee plus the cost of the first class. No gymnastics class refunds or make-ups will be given during the winter session for GRC closings due to extreme weather related conditions. SUMMER CAMP REFUND & CANCELLATION POLICY In the event that you need to cancel a camp, please contact Customer Service immediately at (708) 725-2000. A $25 cancellation fee will apply to refunds for each session of camp that costs over $150 and a $3 cancellation fee for camps less than $150. Once you submit your information, we will set up an account for you, verify your residency if applicable, and send your confirmation via email within two business days. en you can begin registering for classes online! Receipts and class information can be accessed under "My Account." You can always call us at (708) 725-2000 to check your class status, as well! ONLINE REGISTRATION IS FAST, SECURE & EASY! e most convenient way to register is online using our online registration system at https://registration.pdop.org. Aer logging in to your household account, you will be able to view the enrollment status of the class you want, register, and print your own receipt. ere are additional features available when you log in as well, including the ability to print calendars for your household activities as well as pay any outstanding balances. You can add items to your wishlist to remember, compare, or register for at a later time. registration.pdop.org Registration Info Waitlist Policy If you registered and are placed on a waitlist and an opening becomes available, you will receive a phone call and an email requesting payment. Payment must be received by 5:00P the next day or the space will be offered to the next person on the list. Please respond promptly if you are contacted regarding your waitlist status. Pricing: First price listed is for Oak Park residents, second price is for non-residents: $ Res Discount / $ NR Price ($ residents / $ non-residents) Oak Park Conservatory Photography Policy Professional photographers are welcome to use the Oak Park Conservatory facility for photo shoots once the proper permit application has been completed. To acquire a permit application, call the Oak Park Conservatory at (708) 725-2400. Tripods are not allowed when the Conservatory is open to the public. Say Cheese! Photos and videos are periodically taken of people participating in Park District programs and activities. All persons registering for Park District programs/activities or using Park District property, thereby agrees that any photo or video taken by the Park District may be used by the Park District for promotional purposes including its electronic media, videotapes, brochures, flyers and other publications without additional, prior notice or permission and without compensation to the participant.
